Do you need a custom form? I can create a custom form to fit virtually any need. From simple contact forms, address/phone databases, to medical records, and more. All forms can either be hosted on your own server or hosted on one of my secure servers. The data can be sent to you either by a simple e-mail (not recommended for sensitive data) or it can be accessed through an administrative interface.
Do you need a database? If can code a database backend or even an entire turn-key database with data-entry and administrative GUI's. All GUI's are accessible through a simple web interface -- no special hardware or software needed, you simply open a web browser and go!
Do you need a script or other web-based software? If can code and host virtually anything you need. From the simplest, tiniest script to a complex combination of PHP, HTML, SQL, CSS and Javascript.
